Hormone — Androgen

Total Testosterone

Total testosterone measures the full circulating testosterone in serum, both bound and free.

Reference range

Adult male reference: ~264–916 ng/dL (varies by lab and age). Functional optimization context only — not a diagnostic assertion. Range varies by lab, age, and sex.

Why it matters

Total testosterone is the headline androgen marker in any hormone optimization workup. It correlates with body composition, energy, libido, mood, and recovery, and is the first marker a longevity-trained clinician will read against symptoms. In isolation it is incomplete — total testosterone must be interpreted alongside SHBG, free testosterone, and estradiol to understand bioavailability and aromatization. REVRCEL members on a hormone protocol have total testosterone re-read every quarter so dose, frequency, and ancillaries can be titrated against the trend rather than a single snapshot.

How REVRCEL clinicians read it

A REVRCEL clinician reads total testosterone in context — symptoms, SHBG, free testosterone, estradiol, hematocrit — and titrates the protocol over time. A single value is rarely actionable; the trend is.

Common questions

What is total testosterone?

Total testosterone is the sum of all testosterone in serum, both bound to carrier proteins (SHBG and albumin) and unbound (free).

Is total testosterone the same as free testosterone?

No. Total testosterone includes both bound and free testosterone; free testosterone is the small fraction that is biologically active. Both should be measured.

How often should total testosterone be tested on a REVRCEL hormone protocol?

Quarterly. Trend matters more than any single value.
